If you have an insurance provider, different than those listed above, they may allow you to receive care from an out of network provider. Upon your request, we will provide you a clinical receipt, also known as a "superbill", that contains the necessary diagnostic and billing information you need to submit to your insurance company. Depending on your policy, you may be eligible for reimbursement. Please contact your insurance provider to verify your out of network coverage. Payment is due at time of service. We accept Visa, MasterCard, American Express, and Discover. We do not accept cash or checks. Please note that credit card information is required for all prospective patients (including patients with insurance) to finalize appointment booking.
You will be emailed your intake forms, please return no later than 24 hours of requesting your appointment. Forms must be returned same day if requesting an appointment within 24 hours.
Medication Management, Psychotherapy, & Psychoeducation
Approximately 90 minutes. We'll go over your concerns, take a detailed biopsychosocial history, engage in supportive psychotherapy and psychoeducation, make recommendations, and discuss appropriate treatment options. We will also determine if our practice is appropriate for you and your needs. Possible treatment plans may include lab work, referrals for psychological testing, specialty providers, and/or therapy. Also, medications may be prescribed, if appropriate.
Approximately 60 minutes. These appointments are optional for patients that wish to have additional 1:1 time with their provider or would like to combine a therapy session with their medication follow up. Typically, this appointment is required for new problem onset, recent discharge from a higher level of care, clinically complex patients, and family sessions.
Approximately 30 minutes. We will assess your response to treatment, address acute concerns, and engage in supportive psychotherapy and/ or psychoeducation.
Please note that appointment requests are in a tentative status and are not finalized until your intake paperwork is received. We request forms be returned to us within 24 hours of your appointment request (same day if appointment request is for an appointment within 24 hours) to avoid declination of your reservation. Credit card information is collected at time of booking. This is not a comprehensive list of fees. All fees are subject to change.
We require that you cancel your appointment 2 full clinic days prior to your scheduled appointment time. For example, if your appointment is scheduled for Thursday at 10 am ET you must notify us no later than Tuesday at 10 am ET. If your appointment is scheduled on Tuesday at 10 am ET, you must notify us no later than 10 am ET the Friday prior as we are closed on Saturdays and Sundays. Additionally, you must be physically located in the state of Arizona, Maryland, or Washington at the time of your appointment. Failure to show or cancel 2 full clinic days of your scheduled appointment time or failure to be physically present within the serviced state, will result in an automatic charge of the appointment in full (out of network rates).
*Please note that appointments are in Eastern Time (EST/EDT). Failure to show at the appropriate time is considered a no show.
