Though telepsychiatry offers the freedom of receiving care virtually anywhere, providers must be individually licensed in each state. Jade is currently licensed in Arizona, Florida, & Washington. By requesting an appointment below, you confirm that you reside in either of the states listed above.
We are in network with Aetna, Carelon, Cigna, Oscar, Optum/ United, and Oxford.
All appointments are in a tentative status. To finalize your appointment, we require your intake paperwork be returned to us no later than 24 hours of requesting an appointment and same day of your request if appointment is within 24 hours to avoid declination of your request. * Please be sure to attach a photo of your Gov't issued ID.
By requesting your appointment, you agree with the above and confirm that you are 18 years or older, reside in one of the serviced locations, are aware of the insurances we take and our out of network procedures and rates, are aware that appointments are in Eastern Time (EST/EDT), and have reviewed our cancellation policy. You additionally understand that this practice DOES NOT prescribe or continue prescriptions for ANY controlled medications (i.e. Adderall, Xanax, Ambien) in the state of Arizona or Washington.
