All appointments are in a tentative status. To finalize your appointment, we require your credit card information for pre-authorization. After requesting your appointment, a subsequent email will be sent from our secure platform with your patient intake forms. We require these forms be sent back to us same day but no later than 24 hours of your request. Amongst these forms is the credit card authorization. Your card will not be charged until the day of your appointment (for out of network patients). * Please be sure to attach a photo of your Gov't issued ID.
By requesting your appointment, you agree with the above and confirm that you are 18 years or older, reside in one of the serviced locations, are aware of the insurances we take and our out of network procedures and rates, and have reviewed our cancellation policy.
