If you’re a new or prospective patient, the best way to reach us is through our contact form or email. We’re a small practice, so we may not answer live calls — but leave a voicemail with your name, number, and a quick message, and we’ll get back to you within 1–2 clinic days.

Clinic Hours
By appointment only — Monday through Friday, with limited evening slots on Tuesdays.
Admin Hours
Tuesday through Thursday | 10 AM – 3 PM (EST)
This is when we check emails, process forms, and handle non-urgent stuff.
Closed on weekends and all federally recognized holidays.
All appointments are scheduled in Eastern Time (EST/EDT) — regardless of where you are.
We currently provide care in Arizona, Florida, and Washington.
You must be physically located in one of those states during your appointment, or it will be canceled and billed as a late/no-show.

Nope. Jade is a psychiatric nurse practitioner with a doctorate in advanced practice nursing. She assesses, diagnoses, prescribes, and offers therapeutic support — all through care that’s collaborative, thorough, and deeply human.
We ask for at least 48 business hours’ notice if you need to cancel or reschedule. No-shows or late cancels will be charged a $150 fee
Life happens — we get it. But time is reserved just for you, and late changes affect others waiting for care.
We are in network with the following insurance providers:

• Initial Eval (90 mins): $350
• Extended Follow-ups (60 mins): $250
• Follow-ups (30 mins): $175
• Med checks (15 mins): $150
If you’re out-of-network, we can give you a superbill for possible reimbursement through your insurance.
A credit card is required to book. We charge the card on file at the time of service — including any copays, missed appointments, or admin services. Failed payments may result in added fees.
We primarily treat adults 18–64.
We do offer some care for older adults, but more complex geriatric cases may need a specialist referral.

We DO NOT prescribe or continue any controlled medications in Arizona or Washington.
We DO NOT prescribe or continue benzodiazepines (Xanax, Ativan) or controlled sleep meds (Ambien, Lunesta) in any state.
